Good tips. What I think is there should be flexibility in the questions and related materials for the volunteers, so that it may not be rigid and they would get chance to response on their own ways. This can bring lots of hidden things out.

You might look at other customer or employee satisfaction surveys and see if you can tweak them to meet your needs. Here's a basic one:
